Medicare Now Covers E0150 Combination Wheeled Walker/Transport Chair
Rollz Mobility’s devices were the impetus for the change.

Medicare is now covering HCPCS code E0150, defined as “Combination wheeled walker with seat and transport chair, folding, adjustable or fixed height.”

The new durable medical equipment (DME) coverage policy began Oct. 7.

From Oct. 1, 2025, through Sept. 30, 2026, Medicare didn’t cover the code, “as it does not meet the definition of DME,” the Centers for Medicare & Medicaid Services (CMS) said in a policy article revised Oct. 1, 2026. “For dates of service on or after Oct. 1, 2026, a combination wheeled walker with seat and transport chair (E0150) has been determined to meet the definition of DME.”

CMS also said the KX modifier “must be appended to the claim line for the combination wheeled walker with seat and transport chair (E0150) only if all of the standard walker coverage criteria (1-3) in the Walkers LCD (L33791) and the transport chair coverage criteria in the Manual Wheelchair Bases LCD (L33788) are met. Evidence supporting the use of the KX modifier must be retained in the supplier’s files and available to the DME MAC upon request.”

The article explained correct use of the GY modifier, as well: “If a combination wheeled walker with seat and transport chair (E0150) is only needed for mobility outside the home, then the GY modifier must be appended to the code for the item.”

E0150 as a combination mobility device

The E0150 code “describes mobility assistive equipment that integrates the function of multiple types of equipment into a single device,” CMS said. “Code E0150 combines the function of a standard, folding wheeled walker and a transport chair. If the single device does not include the functions of a standard, folding wheeled walker and transport chair, then the device must not be coded as E0150.”

A device with the E0150 code includes the functionalities of transport chairs (HCPCS codes E1037, E1038, and E1039) and standard walkers (HCPCS codes E0141 and E0143).

CMS said Rollz Mobility U.S. Inc. requested a new HCPCS code to define its product, described by CMS as “a rollator and wheelchair combination. It features four large wheels, is heavy duty, foldable, has adjustable handlebars and a solid seat.”

The device, CMS added “allows users to stay as active as possible using the Rollz Motion as a rollator and they can transform it into a wheelchair whenever they need more support. This benefits rehabilitation and achieving a more active lifestyle.”

Applications for E0150

CMS said beneficiaries with multiple sclerosis, Parkinson’s disease, amyotrophic lateral sclerosis (ALS), or other “serious mobility issues” might “benefit greatly from a combination product like the Rollz Motion.”

In documenting its first biannual 2026 HCPCS Coding Cycle for Level II final coding, benefit category and payment determinations, CMS noted that initially, it did not believe there was an existing Medicare DME benefit category for the Rollz Motion product line.

“However, based on additional information provided by the applicant after the coding cycle, we have now determined that these Rollz Motion models are appropriate for use in the home in rare cases where both a walker and transport chair are both medically necessary for use in the home and therefore meet all requirements to be classified as DME.”

The 2026 purchase fee schedule allowables for E0150 are “approximately $135.76, on average, for rural areas, and $89.79, on average, for nonrural areas,” CMS said. “Since the items described by HCPCS Level II code E0150 meet the definition of inexpensive DME at section 414.220(a) of regulations,
payment would be made on a rental or purchase basis.”

Rollz Mobility U.S. Inc., based in Carson, California, offers a series of rollators, including the Rollz Motion Electric, a combination rollator and transport chair operated by joystick. The parent company is based in Delft, the Netherlands.
